Job Description

As a Behavioral Health Professional, you will provide clinical information and education for both inbound and outbound calls, while managing behavioral health crisis calls. You will utilize established protocols along with your independent professional judgment. Additionally, you will document assessments, plans, interventions, and results accurately and effectively.

Responsibilities

* Coordinate daily and participate in ongoing discharge planning for all members admitted at state IMD facilities.

* Attend discharge planning meetings.

* Perform daily post-hospital outreaches for all members admitted to all IMDs with 3 telephonic outreaches within 3 consecutive business days, starting within 24 hours of discharge.

* Perform telephonic post-hospital outreaches for all members admitted inpatient for psychiatric care.

* Coordinate and assist with placement of members that are ED boarding.

* Participate in weekly clinical rounds.

* Meet the 90% monthly TCM audit score.

Essential Skills

* Proficiency in Microsoft Office Suite.

* Excellent verbal and written communication skills.

* Strong time management and timeliness.

* Organizational skills.

* At least 1 year of experience in Behavioral Health in crisis interventions, case management, behavioral interventions, therapy, assessment and intake, or a related area.

* Strong computer skills, including data entry and typing.

* Experience in telephonic counseling and/or a managed care setting is preferred.

Additional

Skills & Qualifications

* Bachelor's degree or equivalent experience;
Master's degree preferred.

* Must have one of the following licenses/certifications:
Licensed Professional Counselor, Licensed Clinical Social Worker, Licensed Marriage and Family Therapist, Licensed Psychologist, Licensed Independent Substance Abuse Counselor, Licensed Associate Counselor, Licensed Associate Substance Abuse Counselor, Licensed Masters Social Worker.

Work Environment

This position is 100% remote and operates Monday through Friday from 8am to 5pm EST.

Job Type & Location

This is a Contract position based out of Bedford, NH.

Pay and Benefits

The pay range for this position is $30.00 - $35.00/hr.
