• Provide clerical and support functions to social work and case management staff
• Assist with patient transfers and discharge processes, ensuring efficient communication and coordination
• Maintain confidentiality while exchanging protected health information with relevant parties
• Support department needs by performing clerical tasks during staff absences
• Participate in quality improvement activities to enhance patient care systems and processes

• Must be 18 years of age with relevant education and/or experience
• Ability to prioritize and organize tasks with attention to detail in a fast-paced environment
• Knowledge of medical terminology and effective relationship-building with healthcare team members
• Excellent verbal communication and telephone skills
• Proficient computer and clerical skills, with familiarity in clinical and business information systems

• High School Diploma or GED
• Associate s or Vocational degree
• 1 year of experience in an acute care hospital setting
