We’re building a care model that empowers clinicians and care teams to do what they do best: provide compassionate, high-impact care. At Pair Team, we leverage AI and automation to reduce administrative burden, streamline coordination, and ensure patients receive timely, personalized support.

Our work is powered by a deeply collaborative team of nurses, social workers, community health workers, and medical professionals working alongside product, technology, and operations to close care gaps and improve outcomes for high-need patients.

We’re one of the largest Enhanced Care Management providers in California and are on track to build the nation’s largest clinically integrated network supporting high-need patients. Our model has demonstrated real impact, including a 58% reduction in emergency department visits and a 29% reduction in hospital admissions
.

At Pair Team, were not just delivering care - we're building the future of more equitable, community-driven healthcare.

About the Opportunity

Pair Team is hiring a full-time Care Coordinator to support our expansion into Chronic Care Management (CCM). This role is a core non-clinical function within our CCM program, focused on providing consistent, relationship-based support for adults managing chronic conditions.

You will partner closely with a Registered Nurse and Nurse Practitioner to support members enrolled in Chronic Care Management, focusing on social needs, care navigation, and referral coordination. This role centers on proactive outreach, resource connection, and ongoing engagement, helping members stay connected to care and the services that support their health between clinical touchpoints.

This is a fully remote position, operating on Pacific Time business hours, with a clearly defined caseload and structured engagement expectations.

What You’ll Do
  • Manage a defined caseload of enrolled CCM (chronic care management) members
  • Conduct proactive outreach to support ongoing engagement and care coordination
  • Identify and address social determinants of health, including food access, housing resources, transportation, and community supports
  • Coordinate referrals to community-based organizations and social service providers
  • Track referral progress and support members in navigating next steps
  • Partner closely with the RN and NP to surface barriers impacting care or engagement
  • Support appointment scheduling, reminders, and follow-up coordination as needed
  • Maintain accurate, timely documentation of member interactions and coordination activities
  • Meet outreach, documentation, and engagement expectations aligned with CCM requirements
  • Collaborate closely with the care team to deliver team-based, whole-person support
What You’ll Need
  • Minimum of 1 year of experience in care coordination, community health work, or a related member-facing support role
  • Bilingual - Spanish and English fluency required
  • At least 1 year of experience working with a Medicare/Medicaid population
  • Experience working in healthcare support call center settings
  • Experience supporting individuals with chronic conditions and ongoing care needs
  • Strong understanding of social determinants of health and community-based resources
  • Comfort working in structured, metrics-driven programs
  • Strong communication skills and ability to build rapport with diverse populations
  • Ability to work independently in a fully remote environment
  • Proficiency with documentation systems, CRMs, or EHRs
  • Reliable internet connection and access to a HIPAA-compliant workspace
Because We Value You
  • Competitive base salary: $25/hour
  • Comprehensive health, vision & dental insurance
  • 401k
  • Monthly work-from-home expense stipend
  • Flexible vacation policy with paid time off
  • Opportunity for rapid career progression and personal growth
  • Work from the comfort of your own home
