Bilingual Care Coordinator (Fully Remote, Spanish Speaking) – full-time

We leverage AI and automation to reduce administrative burden and ensure patients receive timely, personalized care. Our team includes nurses, social workers, community health workers, and professionals in product, technology, and operations to close care gaps and improve outcomes for high-need patients.
We are hiring to support expansion into Chronic Care Management (CCM). This non-clinical role provides consistent, relationship-based support for adults managing chronic conditions, partnering with a Registered Nurse (RN) and Nurse Practitioner (NP).
This is a fully remote position, operating on Pacific Time business hours, with a clearly defined caseload and structured engagement expectations.
What You’ll Do- Manage a defined caseload of enrolled CCM members
- Conduct proactive outreach to support ongoing engagement and care coordination
- Identify and address social determinants of health, including food access, housing resources, transportation, and community supports
- Coordinate referrals to community-based organizations and social service providers
- Track referral progress and support members in navigating next steps
- Partner with the RN and NP to surface barriers impacting care or engagement
- Support appointment scheduling, reminders, and follow-up coordination as needed
- Maintain accurate, timely documentation of member interactions and coordination activities
- Meet outreach, documentation, and engagement expectations aligned with CCM requirements
- Collaborate with the care team to deliver team-based, whole-person support
- Minimum of 1 year of experience in care coordination, community health work, or a related member-facing support role
- Bilingual - Spanish and English fluency required
- At least 1 year of experience working with a Medicare/Medicaid population
- Experience working in healthcare support call center settings
- Experience supporting individuals with chronic conditions and ongoing care needs
- Strong understanding of social determinants of health and community-based resources
- Strong communication skills and ability to build rapport with diverse populations
- Ability to work independently in a fully remote environment
- Proficiency with documentation systems, CRMs, or EHRs
- Reliable internet connection and access to a HIPAA-compliant workspace
