We are recruiting a skilled and compassionate Social Worker to join the Hospital Social Work Team in West Lothian. This is a rewarding opportunity to work within a supportive, experienced, and multidisciplinary hospital-based team, helping adults and their families during times of significant change.

The Role

As a Hospital Social Worker, you will:

* Complete strengths-based assessments to support safe and timely hospital discharge

* Work closely with health colleagues across wards and community services

* Support adults and their carers to make informed decisions about care and support

* Undertake capacity, risk, and safeguarding assessments where required

* Contribute to adult protection work and complex decision-making

* Maintain accurate, high-quality case records and reports

* Promote independence, dignity, and positive outcomes for service users

What We’re Looking For

* Degree or equivalent qualification in Social Work

* SSSC registration (or eligibility)

* Experience in adult social work (hospital or health setting desirable)

* Strong assessment, communication, and multi-disciplinary working skills

* A calm, professional approach in fast-paced environments

* Commitment to person-centred, strengths-based practice
