About the Role
Are you a clinician who prefers the patient's living room to a cramped exam room? We are launching an innovative Home Visit Program to bring high-quality family medicine directly to the Cookeville community, and we are looking for a pioneering NP or PA to lead the charge.
As this is a brand-new initiative, you won’t just be seeing patients; you’ll be helping us build the blueprint. We need a self-starter who is passionate about rural or home-based healthcare and wants to shape how we deliver care outside the traditional clinic walls in Putnam County and the surrounding areas.
Key Responsibilities
- Mobile Primary Care: Conduct comprehensive physical exams, diagnostic screenings, and acute/chronic disease management within patient homes throughout Cookeville and surrounding areas.
- Program Development: Collaborate with leadership to establish protocols, workflows, and scheduling logistics for the new home visit service line.
- Care Coordination: Act as the bridge between homebound patients, their families, and our primary clinic.
- Chronic Care Management (Full-Time Only): Oversee and manage our Chronic Care Management (CCM) program, ensuring high-risk patients receive consistent telephonic or digital touchpoints between visits.
- Patient Advocacy: Navigate social determinants of health (SDOH) by assessing the patient’s home environment and connecting them with necessary resources.
Qualifications
- Licensure: Active, unrestricted NP or PA license in the state of Tennessee.
- Certification: Board Certified in Family Medicine (FNP or PA-C).
- Experience: Minimum of 2 years in family medicine or home health preferred.
- The "Builder" Mindset: Previous experience with program development or clinical leadership is a major plus.
- Logistics: Must have a valid driver's license and a reliable vehicle (mileage reimbursement provided).
- Tech Savvy: Comfortable using mobile EHR systems and managing remote monitoring tools.
