* Job Description:

** We are seeking a compassionate and organized Health Care Coordinator to assist with assessment, care planning, and case management for children and youth with special health care needs and their families. This role supports children and youth with chronic and complex conditions by coordinating services, providing family education, and promoting positive health outcomes.
** Qualifications
* ** Bachelor’s Degree in Social Work, Human Services, Public Health, or related field with:4 years’ experience in care coordination or within community programs serving CSHCN or 6 years of experience working with children & families.
* Preferred:
* Master’s Degree in Social Work, Human Services, Public Health, or related field, with:
* 2 years of experience in care coordination or within community programs serving Children of Special Healthcare Needs (CSHCN) or 4 years of experience working with children & families.
* Effective decision-making skills, demonstration of creativity in problem-solving, and strong communication skills.
* Experience working in early intervention or early childhood services is preferred.
* Valid driver’s license and reliable transportation
* Experience with pediatrics, case management, or care coordination
* Background working with children, youth, and families in healthcare or community-based settings
* Strong communication, organization, and documentation skills
* Knowledge of chronic and complex medical conditions a plus
** Key Responsibilities
*** Assist with assessments and individualized care planning for children and youth with special health care needs
* Provide case management support and service coordination for families and caregivers
* Support and educate families to help reduce caregiver and patient burden
* Collaborate with interdisciplinary teams, providers, and community resources
* Help improve health outcomes while supporting cost-effective care delivery
** Compensation:**$26.10

** Full-time, 40 hour position

Monday - Friday schedule

This is a 40-hour position for the Rochester area. Traveling will be required within New Hampshire. At times, work from home may be an option.
