This is a contract hybrid role for a Nurse Practitioner based in New Jersey state, with provisions for partial work from home. The Nurse Practitioner will conduct patient assessments, diagnose and treat health conditions, and collaborate with care teams to deliver personalized, high-quality care. Responsibilities include providing chronic care management, supporting lifestyle modifications, and coordinating long-term care efforts to ensure optimal patient outcomes.

• Advanced clinical skills in patient assessment, diagnosis, and treatment
• Experience in chronic care management, preventive care, and long-term care coordination
• Strong communication, patient education, and interpersonal skills
• Familiarity with AI-powered tools and technology-driven care management platforms
• Ability to work effectively in a hybrid work environment
• Current Nurse Practitioner license with relevant certifications
• Experience in managing diverse patient populations and delivering personalized care plans
• Knowledge of Medicare reimbursement processes and compliance is a plus
