About Activa Home HealthActiva Home Health is seeking a clinically strong PRN Physical Therapist (PT) to support our Hospital at Home Program. Provide hospital-level PT care in patients' homes focusing on mobility, balance, fall prevention, and discharge readiness.

- Provide skilled physical therapy services to patients receiving hospital-level care at home
- Perform comprehensive PT evaluations addressing:
- Strength, balance, endurance, and functional mobility
- Gait deviations and fall risk
- Transfers, bed mobility, and stair negotiation
- Activity tolerance and cardiopulmonary response
- Develop and implement individualized, evidence-based treatment plans aligned with acute recovery and discharge readiness
- Address functional decline related to:
Recent hospitalization
Acute illness or injury
Deconditioning and mobility loss - Educate patients and caregivers on:
Safe mobility techniques and transfer strategies - Fall prevention and home safety
- Use of assistive devices and mobility equipment
- Monitor patient progress closely, adjust plans of care, and communicate changes promptly with the care team
- Collaborate with physicians, nursing, OT, and care management to support safe transitions and reduce hospital readmissions
- Complete timely, accurate documentation in compliance with Medicare, hospital-at-home, and agency standards
- Active Physical Therapist (PT) license in the state of Florida
- Degree from an accredited Physical Therapy program (DPT preferred)
- Current CPR certification
- Valid driver s license and reliable transportation
- Strong clinical reasoning, communication, and time-management skills
- Experience in one or more of the following:
- Acute care or hospital-based physical therapy
- Hospital at Home or transitional care programs
- Home health physical therapy
- Skilled in:
- Gait training and balance rehabilitation
- Fall risk assessment and prevention
- Functional mobility and discharge planning
- Post-acute and medically complex patient care
- Comfortable practicing independently in a home-based acute care setting
- Familiarity with EMR documentation systems
