- Oversee daily operations related to healthcare ethics, palliative care, and advance care planning (ACP)
- Provide recommendations to enhance ACP for various patient populations
- Monitor and assess departmental and programmatic goals
- Build and maintain relationships with community organizations and stakeholders
- Educate care teams to improve patient experiences with advance care planning
- Master’s degree in a relevant specialty area (social work, nursing, behavioral health, etc.)
- Minimum of 4 years of clinical practice experience in a care setting
- Appropriate licensing/certification related to clinical/care specialty (CNS, LSW, LISW, LCSW, or RN)
- Excellent interpersonal communication and negotiation skills
- Strong analytical, data management, and computer skills
- MSW or BSN degree
- ACP Instructor Certification
- Demonstrated success in improving ACP outcomes in various patient settings
- Ability to influence and motivate teams to adopt new processes
