QualificationsBasic Requirements:
- United States Citizenship:
Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy - English Language Proficiency:
Pharmacists appointed to direct patient care positions must be proficient in spoken and written English in accordance with chapter 2 - section D - paragraph 5a - Education:
Graduate of an Accreditation Council for Pharmacy Education (ACPE) accredited College or School of Pharmacy with a baccalaureate degree in pharmacy (BS Pharmacy) and/or a Doctor of Pharmacy (Pharm.
D.) degree - Verification of approved degree programs may be obtained from the Accreditation Council for Pharmacy Education
- Licensure:
Full - current and unrestricted license to practice pharmacy in a State - Territory - Commonwealth of the United States or the District of Columbia - The pharmacist must maintain current registration if this is a requirement for maintaining full - current - and unrestricted licensure
- A pharmacist who has had licenses revoked/suspended/denied may be appointed only in accordance with VA policy
- Note:
Foreign graduates may meet educational requirements with FPGEC Certification (FPGEE + TOEFL iBT) - Experience:
GS-13 requires 1 year of experience at or equivalent to the next lower grade level (GS-12) with demonstrated medication management and clinical duties - Knowledge, Skills, and Abilities:
Ability to communicate orally and in writing to persuade and influence clinical and management decisions; knowledge of regulatory and quality standards; problem solving and coordination; advanced clinical pharmacy expertise; ability to monitor drug therapies and interpret diagnostics - Preferred Experience:
Chronic disease management, therapeutic drug monitoring, medication therapy management, and patient counseling - Residency training or board certification in pharmacotherapy or related specialty is preferred

Duties and Responsibilities:
Home Based Primary Care (60 to 100% of time) - Conduct comprehensive medication management for Veterans enrolled in the HBPC program, ensuring safe, effective, and evidence-based use of medications
- Collaborate with an interdisciplinary team to develop individualized care plans that optimize therapeutic outcomes and quality of life
- Perform patient assessments, monitor therapeutic responses, and adjust medication regimens accordingly
- Provide education and counseling to patients and caregivers on medication therapy, disease state management, and lifestyle modifications
- Conduct home visits as necessary to evaluate patient adherence and identify medication-related issues
- Ambulatory Care (0 to 40% of time) - depending on HBPC panel size
- Deliver advanced clinical pharmacy services in Ambulatory Care clinics focusing on chronic disease management, anticoagulation therapy, and other specialized areas
- Provide direct patient care through face-to-face or telehealth appointments
- Review and evaluate medication orders for appropriateness and effectiveness; make recommendations for therapy adjustments
- Review nonformulary medication consults to ensure appropriate, cost-effective therapy
- Collaborate with primary care providers and other healthcare professionals to achieve optimal outcomes and safety
- Engage in continuous quality improvement initiatives
- Core Competencies:
