• All opportunities require in‑field visits to patients' homes; remote positions are not available unless specifically noted.

  Per Diem, 15‑20+ visits per week.

  Santa Rosa & surrounding cities.
• Primary location may be reassigned for coverage needs.

As a Clinical Social Worker, you will support patients and their families/caregivers with education and emotional support, focusing on geriatric care that requires understanding local community resources and care planning.

• Assess social and emotional factors related to patients' illnesses and home health needs.
• Develop and implement plans for home social work services, including education of patients, families, and caregivers.
• Coordinate with home health staff and community resources for integrated care.

• Master’s Degree accredited by the Council on Social Work Education.
• Basic Life Support Certification (BLS) required.
• Driver's license and automobile insurance required.
• Experience:
  
  One year as an MSW in a health care setting strongly preferred; home health experience preferred.
• LCSW license highly preferred; supervised licensure provided to eligible candidates.
• Knowledge of local community resources, state and federal homecare regulations, and cultural influences on health care.
• Strong written and verbal communication skills.
• Ability to manage diverse groups, maintain constructive relationships, and prioritize work effectively.
• Commitment to maintaining licensure, certifications, and core competencies.
