Home-Based Primary & Palliative Care | Wisconsin Rapids, WI

Schedule: Monday–Friday, 8:00 AM – 5:00 PM

About the Role:

Are you passionate about providing meaningful, high-quality care to medically complex seniors in the comfort of their homes? We are seeking a dedicated Nurse Practitioner or Physician Assistant (APP) to join a compassionate, interdisciplinary care team delivering primary, urgent, and palliative care to elderly patients.

What You’ll Do:

• Deliver primary and acute medical care in patients’ homes
• Participate in interdisciplinary care planning with physicians, social workers, and paramedics
• Engage patients in ongoing care, wellness planning, and health education
• Address urgent care needs and manage chronic conditions
• Conduct comprehensive assessments, order and interpret diagnostics, and prescribe treatments
• Provide palliative and end-of-life care when appropriate
• Facilitate family meetings and complex care discussions

What We’re Looking For:

• Active NP license and board certification (state-specific)
• Valid driver's license, car insurance, and reliable vehicle for daily travel
• Strong communication skills and the ability to lead difficult clinical conversations
• Home care experience a plus
