Nurse Case Manager

Ascendant Health's mission is to ease the burden of recovery and care-coordination for Michigan’s residents with injuries from auto or work-related accidents. Ascendant Health has a team of skilled nurse case managers to navigate the healthcare system, prevent medical issues, provide choices, tailor treatments, and manage care complexities. Ascendant Health leads with innovative ideas, patient choice, great provider network, and promoting awareness of traumatic brain injury effects.

This is a part-time hybrid Nurse Case Manager role located in the Traverse City area, with some flexibility for remote work. The Nurse Case Manager will coordinate all aspects of injury-related care, advocate for their patients, and develop a care plan to manage care complexities.
