STAR Physical Therapy is seeking motivated and independent Physical Therapists to provide outpatient physical therapy services in the home setting within the Mt Juliet, TN area.
This is a 1099, per diem opportunity offering flexibility, autonomy, and competitive per-visit compensation. Therapists may treat as few or as many patients as they choose and can schedule visits any day of the week.
This role is designed to easily complement an existing outpatient or home health position
, making it ideal for clinicians seeking additional income without the administrative burden of traditional home health.
This position is designed for independent practitioners operating in the home setting around Mt Juliet, TN.
Responsibilities- Accept and manage patients within your designated geographic area
- Evaluate patients and develop individualized plans of care focused on restoring function and independence in the home
- Implement customized treatment programs aligned with physician orders and patient goals
- Communicate effectively with patients, families, physicians, and internal care team members
- Complete timely, accurate documentation for all visits
- Participate in discharge planning and patient education
- Maintain compliance with all clinical and documentation standards
- Outpatient Part B billing model (No OASIS required)
- Fast, efficient EMR system
- Per-visit compensation model with no productivity quotas
- Flexible scheduling including evenings and weekends
- Preference to treat patients close to your primary geographic area
- Mentorship provided for clinicians transitioning from outpatient or home health settings
- Strong referral pipeline supported by a well-established 70+ clinic organization
- Physical Therapist Program Graduate
- TN Physical Therapist License or eligible
- Liability insurance
- Home health experience is preferred but not required — a mentor will assist with documentation and compliance
