Job Summary

As a Remote Advanced Clinical Practitioner, you will work autonomously to provide advanced clinical care to patients across all Archvale practices. You will use your expertise to deliver high-quality care through remote consultations, triage, adhering to clinical guidelines and contributing to the effective management of the practice.

The following are the core responsibilities of the Remote ACP role. There may be occasions where additional tasks are required based on workload and staffing levels:

  • Delivery of highly effective, patient-centered care through remote synchronous and asynchronous consultations.
  • Performing remote triage and prioritising patient needs effectively.
  • Management of acute and long-term conditions, including medication reviews.
  • Processing administration tasks promptly, including referrals, repeat prescription requests, and other related documentation.
  • Maintaining accurate, up-to-date clinical records in line with best practices and policy.
  • Supporting the achievement of practice targets, including QOF indicators.
  • Participating in audits, significant event reviews, and quality improvement initiatives.
  • Collaborating with multidisciplinary teams to ensure coordinated and continuous care.
  • Contributing to the development and implementation of clinical protocols and guidelines.
  • Participating in appraisals, revalidation processes, and continuous professional development.
  • Promoting a culture of safety, learning, and continuous improvement within the practice.
