Position OverviewWe are expanding our management team and seeking a Registered Nurse or Licensed Practical Nurse with extensive experience in home health or elder care. Our ideal candidate is dedicated to delivering top-quality care and is motivated by the desire to make a significant difference in our clients' lives. You will be responsible for coordinating care, managing client needs, and supporting our caregivers.
Responsibilities- Conduct comprehensive initial and ongoing assessments of clients' needs and create individualized care plans.
- Coordinate and monitor total client care, ensuring adherence to best practices and promoting preventative care.
- Develop, review, and update care plans, addressing any client concerns and providing appropriate guidance.
- Follow up on client changes, concerns, and incidents promptly.
- Communicate with family members as needed and supervise and train caregivers on care techniques.
- Provide hands‑on caregiving as necessary and be available for on‑call shifts.
- Audit client charts for compliance and participate in quality improvement activities.
- RN or LPN license in MD or a compact license.
- 5+ years of experience in home health or elder care.
- Knowledge of MD COMAR regulations.
- Strong communication and organizational skills.
- Experience in training and managing caregivers, CNAs, med techs, and other staff.
- Ability to adapt positively and professionally to change.
- Valid Driver's License and Auto Insurance with a clean driving record.
