Care Facilitator​/Medical Assistant

At Home Harmony's Clinical Team is made up of compassionate RNs, LPNs, and MAs who are redefining care for aging adults through a home-based medical and pharmacy model. Were seeking an enthusiastic, organized Care Facilitator – Medical Assistant to play a key role in supporting patients, providers, and community partners as our practice continues to grow.

This is a dynamic, dual-role position ideal for someone who enjoys hands-on patient interaction, care coordination, and building trusted relationships—all while working within a collaborative, mission-driven team.

What You'll Do

Patient & Provider Support (~70%)

• Support telehealth visits in patients homes, assisting with technology setup, clinical needs, and real-time provider communication
• Conduct pre-visit patient check-ins (demographics, medications, allergies, health changes)
• Prepare and upload visit documentation in Athena EMR
• Review hospital discharges and relay key updates to providers
• Participate in daily huddles and interdisciplinary team meetings
• Provide clinical support to partnered senior living communities

Care Coordination & Communication ( ~30%)

• Coordinate scheduling, reminders, and follow-up calls
• Manage incoming patient communications and collaborate across departments (pharmacy, personal care, care programs)
• Track labs, imaging, and referrals to ensure timely follow-up
• Support population health and care coordination initiatives

What We Are Looking For

• Medical Assistant comfortable working in patient homes
• Experience in geriatrics, primary care, internal medicine, cardiac, or pulmonary care preferred
• Passion for working with older adults
• BLS certification required
• EMR experience required (Athena preferred)
• Strong communication, organization, and technology skills
• Friendly, adaptable, and self-directed
• Valid drivers license with clean driving record
• Ability to lift up to 50 lbs and navigate stairs as needed

Schedule & Work Environment

• Monday–Friday, 8:00 AM–4:30 PM
• Office-based during training and probation
• Eligible for partial remote work after probation based on performance and team needs

Compensation & Benefits

• $20–$23/hour, based on experience
