Job SummaryThis position is a STEPS appointment established for a one-year, grant-funded initiative, with the possibility of extension contingent upon continued community service grant funding or University of Kentucky Health Care (UKHC) sustainability efforts.
The Perinatal Community Health Worker (CHW) primary objective is to engage patients during the prenatal period and support Medicaid enrollment and redetermination by facilitating connections between patients and KYnectors. Secondary responsibilities include, but are not limited to: healthcare system navigation; resource identification, management, and linkage informed by social determinants of health (SDoH) screenings documented in the electronic medical record (EMR); building and maintaining culturally responsive relationships with patients and the broader community;
serving as a bridge between medical providers and patients to enhance health literacy; and supporting prenatal and postpartum education aligned with evidence-based care as guided within the EMR.
This role requires adherence to UK College of Medicine professional standards for dress and conduct, HIPAA compliance, completion of required orientations, and compliance with all STEPS employee policies. Limited remote work may be considered as the virtual infrastructure becomes more fully established and contingent upon demonstrated reliability, consistency in performance, and the ability to meet program expectations. Work modality may influence dress code and other operational requirements, as applicable.

- Certified community health worker by the state of Kentucky preferred, required within one year of hire.
- Prenatal Doula/Postpartum Doula experience preferred.
Bilingual preference must have medical interpretation certificate.
