Optum Home & Community Care, part of the United Health Group family of businesses, is creating new integrated care models to address physical, mental, and social needs of patients. In this role you will conduct annual in‑home clinical assessments, provide preventive care, and coordinate follow‑up to improve patient well‑being.
You’ll enjoy flexible scheduling—no on‑call, no weekends, and no holidays required.
This position requires 85% travel within the state of Minnesota, covering all travel expenses. You can live anywhere in MN; the role is primarily a mobile, home‑visit clinical position.
Primary Responsibilities- Conduct in‑home assessments, including past medical history, medication reconciliation, vital signs, and a comprehensive physical exam.
- Perform evidence‑based practice screenings with point‑of‑care testing as appropriate.
- Identify diagnoses for care management and active medical management.
- Develop a list of current and past medical conditions using clinical judgment.
- Communicate findings to the patient’s primary care provider to address gaps in care.
- Recognize urgent or emergent situations and intervene accordingly.
- Educate patients on disease processes, medication use, and compliance.
- Active NP or PA license (or ability to obtain by start date) with no disciplinary actions or restrictions.
- At least 1 year of clinical experience as an NP or PA in a clinical setting.
- Active national certification (ANCC/AANP for NPs or NCCPA for PAs) or ability to obtain by start date.
- Physical ability to lift a 30‑pound bag, navigate stairs, and perform physical assessments.
- Reliable transportation to travel up to 85% of the time within licensed states.
- Experience in gerontology, cardiology, internal medicine, or endocrinology.
- Home health or home‑visit experience.
- Potential to obtain additional licensure in other geographic areas.
Salary ranges from $89,900 to $160,600 annually, based on experience and local labor market. Additional benefits include a comprehensive benefits package, incentive and recognition programs, equity stock purchase, 401(k) contribution, and student loan repayment program. Employees must remain in an advanced practice clinician or physician role for 36 months to receive full student loan repayment benefits, with a minimum 12‑month employment period to maintain regular status.
