CGM & Insulin Pump Referral Coordinator

The CGM & Insulin Pump Referral Coordinator plays a critical role in coordinating and processing referrals for continuous glucose monitors (CGMs) and insulin pumps. This position ensures accurate, timely collection of all required documentation to support durable medical equipment (DME) orders while maintaining compliance with insurance and regulatory requirements. The specialist will serve as a liaison between referral sources, patients, and internal departments to streamline the intake process and ensure high-quality patient care.

Duties

And Responsibilities
  • Process and manage incoming referrals for CGMs and insulin pumps.
  • Review and verify the completeness of patient documentation, including prescriptions, clinical notes, insurance information, and physician orders.
  • Follow up with referring providers and clinics to obtain missing documentation as needed.
  • Act as a primary point of contact for referring offices, prescribers, and internal teams regarding diabetes DME referrals.
  • Coordinate with the insurance verification and authorization teams to ensure timely approvals.
  • Communicate with patients regarding referral status or to gather additional information when required.
  • Ensure all referrals and patient records meet payer-specific and regulatory documentation guidelines.
  • Maintain accurate records in the electronic health record (EHR) and DME software systems.
  • Support audits and quality checks by maintaining organized and compliant documentation.
  • Identify and suggest improvements to referral workflows for efficiency and patient satisfaction.
  • Participate in team huddles, training, and process improvement initiatives.
Basic Qualifications
  • Education:

    High school diploma or equivalent required; associate’s degree or healthcare-related certification preferred.
  • Experience:

    2+ years of experience in healthcare intake, medical referrals, or DME services.
  • Prior experience with diabetes-related DME (CGMs and pumps) highly preferred.

  • Strong understanding of insurance requirements for DME referrals.
  • Excellent communication and organizational skills.
  • Proficient in EHR and DME management software systems.
  • Ability to manage multiple tasks in a fast-paced environment with high attention to detail.
