Position: Nurse Practitioner (Kingston HTT)
Location: Greater London

Overview

Nurse Practitioner Band 6 (RMN) - Kingston Home Treatment Team (KHTT) is a multidisciplinary, community based mental health team providing safe, effective home based assessment and treatment as an alternative to inpatient admission and for early discharge from the ward in the least disruptive environment. The workload is team-based. The post holder will be based at Tolworth Hospital and work across the borough of Kingston with shifts required.

The post holder must have a valid driving licence and access to a car to enable home visits. This role offers flexible and agile working with development and career progression opportunities.

Job responsibilities
  • Be the first point of contact for all referrals within the Home Treatment Team (CHTT).
  • Be directly accountable for care planned and implemented within the limits of professional competence.
  • Develop and maintain high standards of practice based on assessment, psycho-education, the Recovery Model, and supportive clinical management.
  • Communicate and liaise effectively with relevant agencies and teams to provide an integrated programme of care.
  • Attend clinical meetings to provide information on caseloads and new assessments; feedback on zoning.
  • Identify and support carers and facilitate assessments where appropriate.
  • Offer advice and support to other professionals on referrals to HTT or signposting if not appropriate for HTT.
  • Undertake home visits (single or joint) where no health, risk or safety issues are evident.
  • Actively participate in relapse prevention and support clients in self-management and collaborative crisis planning.
  • Provide comprehensive risk assessments in accordance with Trust guidelines.
  • Ensure KPIs are assessed, recorded and updated on RIO; work flexibly to engage service users and carers.
  • Maintain effective communication with inpatient services, social services, CMHTs, and other statutory/non-statutory organisations, including the voluntary sector, to ensure continuity of care.
  • Have knowledge of the Mental Health Act, Mental Capacity Act, Care Programme Approach, Care Act, Deprivation of Liberty Safeguards, Safeguarding Vulnerable Adults, and Community Treatment Orders.
  • Maintain confidentiality and accurate records in line with NMC guidelines and Trust policy.
  • Manage medicines in accordance with NMC guidelines and the Trust Medicines Policy.
  • Coordinate with Children & Families Teams on safeguarding children.
  • Report incidents or Serious Untoward Incidents to the Team Manager.
Person specification

Essential

  • RMN with current NMC registration
  • Evidence of continuing professional and personal development
  • Full current driving licence and car owner (or willingness to join the Trust car lease scheme)

Desirable

  • Mentorship (ENB 998 or equivalent)
  • RGN
  • Diploma in cognitive behavioural therapy/psychodynamic counselling

Experience

  • Demonstrable experience of working with people with serious ongoing mental health problems in community/outpatient settings post-qualification
  • Experience in a multidisciplinary team
  • Experience of assessing and managing crisis in severe mental illness
  • Experience of clinical supervision

Desirable

  • Experience in managing and supervising staff
  • Experience contributing to policy and procedure development and implementation

  • Disclosure and Barring Service (DBS) check is applicable; post may require DBS disclosure
