Compensation: $35–$40/hour + paid time off and training reimbursement

Schedule: Primarily in-person with some remote work options. In-person services provided at our Clinic during business hours (M-F 9am-5pm); some options for telehealth provision for clients with digital access both during and outside of regular business hours; attend required meetings

You’ll provide direct therapy services and support people in navigating resources, whether that’s helping with housing applications, connecting them to community support, or coordinating with other providers to ensure needs beyond therapy are met.

This is hands on work that blends clinical expertise with practical, person-centered problem-solving.

We’re a community mental health center serving adults in the Capitol Hill area of Denver. Our clinic and drop-in center use trauma-informed and recovery-oriented practices to support members experiencing severe and persistent mental illness. • Direct Clinical Services & Case Management
• Provide clinical screenings, intake assessments, diagnoses, and treatment for adults with severe and persistent mental illness using evidence-based approaches.
• Develop and implement individualized treatment plans in collaboration with clients, grounded in their goals and strengths.
• Carry a manageable caseload of 35–40 adults, maintaining high-quality, person centered care.
• Offer individual and group therapy that supports recovery, self-advocacy, and community connection.
• Provide case management and resource navigation to address needs beyond therapy, including housing, benefits, medical care, and other supportive services.
• Work closely with the Clinic Team Lead, Psychiatric Nurse Practitioner, and other team members to ensure integrated care.
• Actively connect clients with community resources and other Heartland programs to support whole-person wellbeing.
• Consult with external providers for coordination of services and continuity of care.

• Identify urgent mental health conditions and intervene using trauma-informed de escalation strategies.
• Make appropriate referrals to emergency or psychiatric treatment facilities when necessary.

• Provide clinical supervision to master’s level intern(s) and participate in intern recruitment, training, and evaluation.
• Collaborate with the Clinic Team Lead to support program goals and development.

• Maintain accurate, timely documentation for all services, assessments, and treatment goals in compliance with medical record standards.
• Track billing information and ensure adherence to all state, federal, and Heartland policies.

• Master’s degree in social work, counseling, or psychology.
• Current Colorado licensure as an LCSW or LPC in good standing.
• Minimum 1 year of outpatient mental health experience; 1–3 years preferred.
• Experience working with adults with severe and persistent mental illness.
• Knowledge of behavioral health needs of adults with serious mental illness.
• Knowledge of trauma-informed principles and practices.
• Experience providing both individual and group therapy.
• Skill in establishing and maintaining productive relationships with clients and their support systems.
• Ability to assess crisis situations and intervene appropriately.
