Overview
Bolton Living Well currently have a full-time post available on a fixed-term or secondment basis for a maximum period of 12 months. We are looking to recruit a Registered Mental Health Nurse, Registered Social Worker or Occupational Therapist to fill this role. The postholder will undertake a key worker role, alongside an assessment and formulation role within the team. The Living Well Service enables timely access to mental health services when people need it most and provides support for people who might not traditionally meet the threshold or criteria of existing mental health services.
The team is multidisciplinary and is operated in partnership with Family Action (BAND), our third sector partner.
The postholder will be based in either of our Hubs, in either Farnworth or Horwich and subject to clinical need. The service operates from 9am to 5pm Monday to Friday, however staff will be expected to participate in a Duty capacity on a rota basis where they will be situated within the new Referral and Assessment Hub based at the Royal Bolton Hospital on those days they are rostered to support the rota.
Staff may be expected to work until 7pm on those days, dependent on service need and may include occasional weekend working as the service develops.
- Support a multi-disciplinary team by undertaking key worker responsibilities and contributing to assessment and formulation processes.
- Triaging and signposting referrals and liaising with a range of agencies and professionals.
- Develop knowledge of community services and third sector organisations in Bolton to inform referrals and care planning.
You will need to have experience of triaging and signposting referrals as well as being able to liaise with a number of agencies and professionals. Experience and knowledge of community services and third sector organisations in Bolton will be beneficial for this role.
