Position SummaryThe Licensed Practical Nurse (LPN)/Registered Nurse (RN) Care Coordinator supports patient-provider communication by managing telephone encounters, triage patient calls, relaying results, and providing education and care coordination. This role offers flexibility to work remotely and onsite, improving patient access and care continuity. This position follows a Monday through Friday schedule and is hybrid, with the ability to work remotely as approved.
Key Responsibilities- Answer and triage patient calls based on clinical urgency.
- Respond to all nurse triage events for walk in patients.
- Document patient symptoms and interactions clearly in eClinical
Works. - Coordinate provider responses and relay instructions to patients.
- Create, manage, and close telephone encounters promptly.
- Schedule patient appointments as needed.
- Relay lab and diagnostic results to patients as directed by providers in a timely manner.
- Assist with prescription refills, prior authorizations, and referrals.
- Educate patients about managing acute and chronic conditions.
- Support follow-up scheduling and care gap closure; assist with chronic care management.
- Always Protect patient confidentiality (HIPAA compliance).
- Participate in clinical quality improvement and team meetings.
- Post op call backs (check on patients who had a difficult procedure to see how they’re doing).
- Provide back-up support to clinical staff, as needed.
- Current LPN or RN license in Virginia.
- CPR certification.
- Experience with telephone triage is preferred.
- Proficiency in Electronic Health Records (eClinical
Works preferred). - Reliable internet connection for remote work.

- Experience working in an FQHC, outpatient clinic, or family medicine practice.
- Formal training or certification in Health Information Management (HIM) a plus.
- Demonstrates strong verbal and written communication skills.
- Exercises sound clinical judgment with effective triage capabilities.
- Manages multiple tasks efficiently while working independently.
- Maintains high attention to detail with excellent organizational skills.
- Handles sensitive and confidential information with discretion.
- Excels in time management and thrives in deadline-driven environments.
