Home-based hospice RN Case Manager providing skilled nursing, symptom management, and coordinated interdisciplinary care for terminally ill patients in the home or skilled nursing settings. Responsibilities include comprehensive assessment, individualized care planning, implementation and documentation in the EMR, communication with patients/families and the care team, and on-call/coverage across regions. Position requires an active RN license, valid driver’s license, IV and assessment skills, and willingness to participate in weekend/on-call rotation and travel within the service area.

Hospice, the final stage of BJC's continuum of care, is a special kind of caring for patients with a life-limiting illness. Services are provided in the comfort of the patient's home or skilled nursing facility. We provide a multi-disciplinary team of healthcare professionals and volunteers, specially trained in symptom management, pain control, counseling and bereavement services for the dying.

Our Hospice services include alternative therapies such as music, art and massage therapy. Our Hospice programs provide palliative care by helping patients manage their pain and symptoms while living their lives with daily peace and dignity.

Role Purpose

Evaluates the client and furnishes services requiring substantial and specialized skill, appropriate preventive and rehabilitative nursing procedures, and instructions to assist the client in learning appropriate self-care techniques. When assigned as case manager, the staff nurse is responsible for coordinating all aspects of care related to that patient.
