Service Coordinator – Madison, Onondaga, Oneida, Oswego, and Herkimer counties

The Service Coordinator assists participants in the development, implementation, and monitoring of all services in the Service Plan. Services may include Medicaid State Plan services, non‑Medicaid federal, state, and locally funded services, as well as educational, vocational, social, and medical services. The goal is to increase the participant’s independence, productivity, and integration into the community while maintaining the health and welfare of the individual.

Roles and Responsibilities
  • Assist the prospective participant to become a waiver participant
  • Update and submit timely Service Plans
  • Support the participant to become an effective self‑advocate and problem solver
  • Organize and facilitate Team Meetings
  • Maintain records for at least seven years after termination of waiver services
  • Ensure that the Level of Care (LOC) is completed annually and as needed
  • Travel to monthly face‑to‑face meetings with participants
  • Maintain ongoing communication with the participants’ service team on a monthly basis to ensure satisfaction of services and safety
  • Maintain records of waiver transportation

Qualifications

To be eligible for this position, applicants must:

  • Have a valid NYS Driver’s license and the ability to travel to participants’ homes.
  • Be able to work independently to submit timely documentation required to complete SC job tasks.
  • Have the ability to manage community resources to build Service Plans and assist with SC training and resources for the team.
  • Manage a full caseload, understand Waiver rules, and educate others on those rules.

In addition, one of the following conditions must be met:

  • Persons self‑employed or employed as Service Coordinators must hold
    • Licensed Master Social Worker, Licensed Clinical Social Worker (NYS Education Department);
    • Individual with a Doctorate or Master of Social Work;
    • Individual with a Doctorate or Master of Psychology;
    • Individual with a Master of Gerontology;
    • Physical Therapist (Licensed by NYS);
    • Registered Professional Nurse (Licensed by NYS);
    • Certified Teacher of Students with Disabilities (certified by NYS);
    • Certified Rehabilitation Counselor (Commission of Rehabilitation Counselor Certification);
    • Licensed Speech Pathologist (Licensed by NYS);
    • Occupational Therapist (Licensed by NYS).
  • The above qualified candidate must also have at least one (1) year of experience providing case management/service coordination and referral of community‑based services for individuals with disabilities and/or seniors.
  • Alternatively, a Bachelor’s degree holder must have three (3) years of experience providing the same services.
  • An Associate’s degree holder must have five (5) years of experience providing the same services.
Travel

Ability to travel frequently to client homes throughout Madison, Onondaga, Oneida, Oswego, and Herkimer counties. Ability to travel to Venture Forthe’s Syracuse office as needed and to the Buffalo office for initial training.
