Job Title
Licensed COTA – In-Home Functional Skills & Independence Coach
Compensation$42–$46 per visit

- Maintain timely, clear documentation to promote strong clinical outcomes
- Current Minnesota Certified Occupational Therapy Assistant (COTA) license
- Valid driver’s license and insured, reliable transportation
- Strong communication skills and a passion for patient‑centered care
- Home health experience preferred, but supportive training is available
