OverviewAs the Social Services Director you will identify and provide for each resident’s social, emotional and psychological needs, and the continuing development of the resident’s full potential during his/her stay at the facility and to assist in the planning for his/her discharge.
How you Will make a difference- Assist residents and their families with social, emotional, and family issues.
- Provide timely and accurate completion of documentation needed to comply with Facility, State, and Federal policies.
- Participate in resident care planning.
- Participate in the development of Discharge planning and prepare resident for the transition.
- Serve as a general source of information for residents and their families.
- Bachelor’s or Masters Degree in Social Work or Psychology.
- 3+ Years Experience working in Social Services, directly with individuals, preferably in a healthcare setting.
- High level of professionalism and ability to maintain confidentiality.
Bachelor's Degree in Social Work or Psychology REQUIRED!
