Overview
PASSPORT Case Manager / Waiver Service Coordinator — Jefferson County and/or Northern Tuscarawas County – Dover/New Philadelphia area.
The Area Agency on Aging, Region 9, is dedicated to helping older adults and people with disabilities remain happy, healthy, and at home in the counties we serve.
Responsibilities- Evaluate members’ needs and enhance their overall wellness through home visits with face-to-face assessments.
- Licensed Social Worker (LCSW) or Registered Nurse (RN).
- Self-motivated, energetic, detail oriented, highly organized, tech-savvy.
- Must live in Jefferson County and/or Northern Tuscarawas County and have reliable transportation, valid driver’s license, and proof of automobile insurance.
- Full-time remote position with starting salary of $57,644.
- Flexible hours Monday through Friday; remote schedules; well-developed remote training program; generous paid time off.
- Pre-employment drug/alcohol testing and background check required.
