Community Care Coordinator - Region Remote

• Provide proactive patient support and coordinate care delivery for patients with amputations and limb differences
• Assist patients and families with information, resources, and emotional support throughout the treatment process
• Build and maintain relationships with referral sources and healthcare providers to enhance patient care
• Ensure timely follow-up communications and appointments for optimal patient experience and continuity of care
• Act as a liaison between patients and various clinic areas to facilitate pre-operative and post-operative planning

• Minimum of 2 years of experience in a patient care environment
• BA/BS in a health-related field or equivalent work experience
• Ability to be credentialed in major hospital systems
• Strong customer service skills in a fast-paced environment, including handling confidential data discreetly
• Valid driver's license and a driving record that meets the employer’s safety policy standards

• Certifications such as ATC, EMT, MA, Surgical Tech, LVN, PTA, Physician Extenders, OTA, or Social Work
• Experience in healthcare facility administrative support functions such as insurance processing or patient care coordination
• Familiarity with medical reimbursement and terminology
• Experience working directly with healthcare professionals including physicians and therapists
• Proficiency with SharePoint and CRM systems
