Position: Clinical Coordinator-START Program

START CSIDD Coordinator

Full‑time, remote‑friendly position based in Buffalo, NY. Work from home flexibility that lets you make a positive impact while staying in your own space.

Salary: $60,405.80 - $65,405.80 per year, with a $5,000 sign‑on bonus. An additional $5,000 annual pay increase will be applied after successful completion of START Certification.

Benefits include Medical, Dental, Vision, Life Insurance, Health Savings Account, EAP Program, Paid Time Off, and mileage reimbursement at the current federal rate.

Responsibilities
  • Maintain an active caseload of individuals referred for START services.
  • Develop cross‑system plans and provide outreach, consultation, and crisis supports.
  • Coordinate therapeutic resource center services, in‑home support, crisis response, consultation, education, and training across 17 counties in Western New York.
  • Engage families and community partners to ensure person‑centered solutions that allow residents to stay in their homes or community residences.
  • Manage frequent travel throughout the Buffalo area for field visits and consultations.
  • Document services, write reports, and communicate progress to stakeholders.
Qualifications
  • Master’s degree in Social Work, Psychology, Counseling, or another Human Services field.
  • At least 2 years of experience working with individuals with intellectual/developmental disabilities (IDD) and mental/behavioral health needs.
  • Familiarity with complex systems, social work practices, mental health, and family dynamics.
  • Strong organizational skills to manage a caseload effectively.
  • Excellent written and verbal communication skills for outreach and consultation.
  • Customer‑centric mindset and a proven ability to solve problems empathetically.
