In home dialysis, how care is delivered matters just as much as the care itself. While Registered Nurses oversee clinical assessment and treatment, Patient Care Technicians (PCTs) are often the ones patients see, hear, and connect with most. You are the calm presence in the home - the person who explains, reassures, observes, and ensures patients truly understand what’s happening. This is not a clinic-based role.
It is a field-based, patient-facing position supporting dialysis patients in their homes within a defined territory. This job is designed for someone who brings empathy, emotional intelligence, and follow-through into every home they enter. Technical skills can be taught. Care, awareness, and human connection cannot.

- Patient Education & Support
- Reinforce dialysis education provided by nursing staff
- Educate patients and caregivers using approved materials and protocols
- Confirm comprehension using teach-back and observation
- Provide compassionate, patient-centered support in the home
- Identify and communicate concerns or changes to supervising RNRN
- Technical & Dialysis Support
- Travel between patient homes within assigned territory
- Support dialysis equipment setup and breakdown (within training/scope)
- Perform blood draws and specimen collection per protocol
- Assist with troubleshooting equipment issues as trained
- Documentation & Collaboration
- Document patient interactions and education in the EMR
- Communicate timely updates to clinical leadership
- Support care coordination and continuity
- Maintain compliance with all policies and procedures
Required
- Patient Minimum 1.5 years of prior dialysis experience
- Documented patient education responsibilities as part of prior role (examples include EMR documentation, care plans, education notes, patient onboarding, modality education, or training records)
- High School Diploma or GED
- Valid Florida Driver’s License
- Ability to travel between patient homes throughout the workday
- High level of professionalism and customer service orientation
- Experience using Electronic Medical Records (EMR) systems
- Comfortable documenting patient education and interactions
- Proficient with basic computer systems (Outlook, Word; Excel helpful)
- Strong written and verbal communication skills
Preferred
- Experience in home dialysis (HHD or PD)
- Experience educating patients in dialysis, nephrology, or chronic disease
- Prior field-based or in-home healthcare experience
- Familiarity with DaVita, Fresenius (FMC), or similar dialysis environments
- Bilingual (English/Spanish or English/Creole) a plus
- One assigned territory — no floating
