• Coordinate care for clients with behavioral health conditions requiring intensive interventions and oversight
• Conduct in-depth health risk and comprehensive needs assessments
• Develop and communicate care plans, ensuring services are rendered appropriately
• Monitor and evaluate the effectiveness of care plans and interventions
• Collaborate with interdisciplinary teams to address care issues and improve member satisfaction

• 3-5 years of experience in Social Work, Nursing, or a healthcare-related field, or relevant experience in lieu of a degree
• Experience in utilization management, quality assurance, or community health
• Strong analytical skills for trend analysis and decision support
• Effective verbal and written communication skills
• Valid driver s license in the state of residence

• Associate or Bachelor s degree in a related field
• Certification as a Case Manager (CCM) or Licensed Clinical Social Worker (LCSW)
• Experience in business management, including cost/benefit analysis and negotiation
• Knowledge of referral coordination to community and private resources
• Familiarity with care coordination processes and policies
