Job Title:

Care Manager

Location:

Northshore of Boston area, with potential travel to other communities. This position is part-time (10-20 hrs/wk) with potential to grow to full time. Hybrid position with some remote/work-from-home and some field work.

Responsibilities
  • Needs Assessment:
    Evaluating a client’s physical, psychological, and social needs.
  • Care Planning:
    Developing and implementing individualized care plans based on assessed needs.
  • Coordination of Services:
    Connecting clients with appropriate healthcare providers, community resources, and support systems.
  • Communication:
    Facilitating clear communication between clients, families, and healthcare professionals.
  • Monitoring and Evaluation:
    Tracking client progress, assessing the effectiveness of care plans, and adjusting as needed.
  • Advocacy:
    Representing the client’s best interests and ensuring their voice is heard within the healthcare system.
  • Education and Support:
    Providing information and support to clients and their families regarding their condition, treatment, and available resources.
  • Documentation:
    Maintaining accurate and up-to-date client records and care plans, including billing/EMR entry within 24 hours of the activity (same-day preferred).
  • Caseload:
    Maintaining an active client caseload to drive billable hours.
  • Community:
    Building relationships with community service providers and elder care professionals, attending meetings as needed, and generating referrals for the organization’s business growth.
  • Goal-oriented Feedback:
    Proactively evaluating client goals and steps needed to reach them, providing feedback.
  • Field

    Activities:

    Performing home visits, ER/hospital visits, and attending MD appointments as needed.
  • Other duties as assigned by supervisor.
Qualifications
  • Strong ability to assess clients’ needs and make recommendations accordingly.
  • Strong interpersonal and communication skills (verbal and written) and ability to communicate effectively with physicians regarding medical concerns.
  • Excellent organizational and time management skills.
  • Ability to work independently and as part of a team.
  • Ability to work remotely and travel to clients’ locations or appointments.
  • Knowledge of healthcare systems and community resources, entitlements, and health insurance options.
  • Experience in a clinical setting; medical case management or care coordination experience preferred.
  • Exhibit empathy and compassion.
  • Problem-solving and critical thinking skills; able to think "outside the box".
  • Proficiency in using electronic health records (EHRs) and other healthcare technology.
  • Ability to work in a non-traditional professional environment and manage fee-for-service caseload effectively.
  • Previous experience in elder care (5+ years) preferred.
  • Medical terminology and knowledge of common diseases related to aging.
  • Ability to manage challenging family dynamics and complex situations.
  • Knowledge and experience working with Alzheimer’s and related disorders and/or mental health disorders.
  • Ability to obtain and/or maintain professional licensure as well as ALC (as applicable).
  • Education and Experience:

    Bachelor’s degree or higher in a related field. MSW/LICSW or RN preferred. Other licensure considered (e.g., PT, OT, SLP, etc.).
  • Must have a valid driver’s license and own a reliable car with appropriate car insurance. Must have valid right to work in the United States.
