Supported Assistant Practitioner/Assistant Practitioner | 9711 | permanent contract | 37 hours per week | £28,598 to £33,699 per annum | Grade F/G | Kings Lynn

We are seeking an enthusiastic Assistant Practitioner to join our team, supporting people in the community following their discharge from hospital. You will visit individuals in their own homes, residential and nursing settings, and hospital wards to help them achieve their desired outcomes and promote independence through a strengths-based approach.

You will need excellent communication, record keeping, and keyboard skills. Strong organisational skills are essential to manage the fast-paced nature of our work, as assessments must be completed in a timely manner.

This role sits within the West Discharge Follow On Team on a full time basis. The team operates a hybrid working model, combining office based and remote working.

A full driving licence and access to a vehicle are required, as our team covers all of West Norfolk. You may also occasionally be required to visit individuals in other areas of Norfolk, including locations not easily accessible by public transport.

You will be joining a highly supportive and dedicated team, so being a collaborative team player is essential. You must also be confident working independently and making appropriate decisions.

Ideally, you will have experience working in social care. However, for the right candidate, we can offer the opportunity to join the team through the supported Assistant Practitioner programme.

This is an exciting time to join Norfolk County Council as we transition to a new way of working with people in the community under the Human Learning Systems approach. This model moves away from process driven “hand offs” and instead focuses on building genuine connections with local networks and community support. By fostering meaningful interactions, we aim to enhance the quality of care and support available in our neighbourhoods and communities.

Our Supported Assistant Practitioners (Scale

F) join our 9‑month, bespoke support programme enabling them to progress to an Assistant Practitioner role (Scale G) subject to satisfactory programme completion.
