• Primarily support in-home, patient-facing visits to support patient care needs in the assigned market.
• Partner with other clinicians in the Care Community managing the health and wellbeing of patients in their home settings.
• Engage attributed patients into the Well Be program and support care needs and efforts to keep patients engaged.
• Facilitate staff, patient, and family decision-making by providing educational resources.
• Perform initial patient screenings/physical examinations, create assessment/plan and complete documentation in a timely manner.
