Salary Range: $72,883 - $82,463
The Town of Brookline Council on Aging is seeking an experienced and compassionate Clinical Social Worker II to provide administrative, supervisory, and clinical casework services to Brookline residents age 60 and older.
Responsibilities- Provide outreach and home-based clinical and case management services, conduct psychosocial assessments, and develop service plans to address client goals and needs.
- Administer the innovative Homecare Program and make referrals for medical, social, housing, legal, and financial services.
- Provide crisis intervention, including suicide risk assessments and safety planning, and investigate referrals regarding elders at risk, abuse, or self-neglect in coordination with state and local agencies.
- Advocate and liaise with health care providers, social service agencies, housing providers, and other community organizations.
- Participate in interagency meetings, represent the Council at community events, assist with program development, and contribute to resource materials.
- Provide day-to-day supervision to volunteers, maintain confidential client records, and prepare reports.
- Based at the Brookline Senior Center but with frequent home visits to older adult residents; requires local travel and flexibility to work outside regularly scheduled hours; perform other related work as required.
- Master’s in Social Work required with expertise in Gerontology and two years related experience; L.I.C.S.W. required.
- The ideal candidate will have a deep understanding of the services and resources available to older adults, along with strong knowledge of state and federal regulations that impact this population.
- Skilled in counseling, casework, and crisis intervention; able to support individuals and families with compassion, empathy, and patience.
- Strong communication, organizational, and time management abilities; capacity to coordinate care effectively while maintaining professional boundaries and confidentiality.
- Proficiency with database and word processing applications for managing records and reports.
