Native American Community Clinic
LocationMinneapolis, MN (Remote & Work from Anywhere available)
Pay Range$64,480 – $85,280 per year (base pay; actual pay will be based on skills and experience).
Job SummaryThis position supports the NACC Community Health Department by managing preventive care and chronic disease management grant programming and quality improvement initiatives through patient outreach and education. The Registered Dietitian will assess nutritional status and develop appropriate care plans for patients in outpatient and community settings.
Essential Duties and Responsibilities- Assess patients’ nutritional status and needs, evaluate and chart results of nutrition counseling using the Nutrition Care Process.
- Provide individual and group nutrition counseling, including goal setting.
- Actively participate in integrated care team with medical providers, community pharmacists and other healthcare professionals to coordinate medical and nutritional needs.
- Manage chronic condition population patient panels through registry monitoring and patient outreach following clinical protocols.
- Maintain timely follow-up care with all nutrition referrals and appointments.
- Act as a nutrition information resource for medical providers, nursing and other clinic personnel, and the community.
- Deliver community nutrition programs.
- Develop, acquire and maintain culturally appropriate nutrition education materials.
- Phone and mail outreach for population health management, including scheduling appointments and basic care coordination.
- Recruit for chronic disease management groups and preventative care events and programming.
- Support grant specific program evaluation activities and incorporate findings to refine and improve program effectiveness to meet grant deliverables.
- Work with grant/project evaluators and Community Health Manager to develop data and reports as required to fulfill evaluation plans.
- Maintain regular communication with project partners and the project officer to fulfill grant requirements, including submitting reports as required.
- Lead employee wellness initiatives via the Health Equity Committee with support from Community Health Manager.
- Support ongoing sustainability planning on behalf of Community Health Programs.
- Play an active role on the outreach team. Other duties may be assigned.
- GREAT LAKES INTERTRIBAL EPIDEMIOLOGY CENTER (GLITEC) GOOD HEALTH AND WELLNESS IN INDIAN COUNTRY GRANT:
- Support local food and beverage programs and systems by helping navigate and refer to community gardens and Indigenous Food Network partners.
- Support breastfeeding services within existing clinic and community support services.
- Increase awareness and screening of prediabetes among tribal members and health care providers.
- Educate and raise awareness of chronic disease prevention and management at urban Native American community events.
- Engage patients in regular screening of chronic diseases including diabetes.
- Provide lifestyle support for diabetes prevention and prediabetes.
- Link resources and clinical services to support prevention, detection, and control of high blood pressure or high blood cholesterol.
- Engage in culturally specific chronic disease prevention education and production.
- Directly deliver patient education and support.
- PROOF ALLIANCE MATERNAL AND CHILD HEALTH GRANT:
- Serve women and their families by providing culturally-informed evidence-based services and supports.
- Conduct culturally-relevant education and programs to increase knowledge, resources, and social support.
- Provide traditional and culturally relevant patient and community education on pregnancy and motherhood.
- Coordinate referrals for participants with partners offering culturally specific maternal-child health programming.
This job has no supervisory responsibilities.
Qualifications- Education and/Or Experience
- Bachelor or Master Degree in Dietetics
- Registered Dietitian (RD) from Commission of Dietetic Registration required
- Licensed Dietitian (LD) from MN Board of Dietetics and Nutrition Practice required
