Position: Medical Social Worker New Hingham, MA

We are looking for an experienced, talented, and compassionate Hospice Social Worker for a part-time position in Quincy, MA.

Responsibilities
  • The Hospice Social Worker will work as part of a home-based interdisciplinary team providing counseling and targeted interventions for a population of homebound patients and their caregivers who are receiving palliative or hospice services through Croí Health.
  • Conduct an initial psychosocial assessment of the patient/caregiver contributing to the Agencys plan of care.
  • The Hospice Social Worker strives for optimal patient outcomes with improved functioning and increased ability to self-manage bio-psych-social challenges.
  • Provide social work services reflective of the philosophy, mission, and standards established by the Agency.
What we’re looking for in you
  • Current MA license in good standing
  • Bachelors degree in counseling or closely related field
  • Master of Social Work (MSW) from an accredited program
  • 3+ years of social work experience
  • Hospice work a plus
  • Previous community health and medical health experience preferred
  • In-depth knowledge of social service skills and resources
  • Good verbal and written communication skills
  • Ability to interact harmoniously with patients/clients, families, and colleagues
  • Ability to work under pressure in various environments and in the field
  • Ability to work independently with minimal supervision
  • Excellent organizational, flexibility, and time management skills
  • Keeping up to date with education, policies, and procedures
  • Ability to cover the full South Shore field and, in the future, focus on one central area
  • Flexibility for on-call holiday and weekend rotation

Additional information

There is a weekend/holiday structure that is shared amongst the team using an on-call method. This role requires hours located on the South Shore, which may include Milton down to Carver. In the future this may be scaled back to one concentrated area.
