We are looking for an experienced, motivated and caring health care assistant to join our friendly Team. We pride ourselves on providing outstanding care to patients in the Worcester City area. Working within the Neighbourhood team is both rewarding and varied, giving you the opportunity to widen your skill base and achieve great job satisfaction.
The Neighbourhood Team operates from 07:30-18:30 each day and you will work 07:30-15:30, 08:30-16:30 or 10:30-18:30 shift patterns on a roster basis.
Our Neighbourhood team consists of Community Nurses, Physiotherapists, Occupational Therapists, Healthcare Assistants and administrative staff; with GP and social care colleagues working closely alongside.

Please note the Trust will not routinely provide sponsorship to work for this role. In addition to meet Home Office salary threshold you must have the significant experience to be offered top of the pay band AND work a minimum of 35.5 hours per week. The Trust will not provide sponsorship to work for this role, and employment on a time limited visa doesnt guarantee sponsorship at point of expiry.
Mainduties of the job
While on duty you will either be working alone or alongside colleagues to support delegated tasks in line with Trust standards and core values. Each task will have been assessed by a qualified health care professional and a care plan will be in place for you to follow and report back on. Tasks may include personal care, supporting daily activities, participating in manual handling and other allocated tasks such as dressing wounds, setting up equipment, and recording observations.
All clinical documentation is recorded in line with Trust guidelines and you will give daily handovers to team leads summarising your involvement with your allocated patients. Clear and timely communication is key to your role which may be in a verbal, non-verbal or written format to support patient centred care.

Maintaining accurate and contemporaneous records in accordance with Trust policies/procedures; while maintaining confidentiality at all times.

Provide a high standard of outcome orientated care and support following a care/recovery plan under the supervision and guidance of qualified health care professionals
Maintaining effective communication systems within the integrated team, with other agencies, the voluntary sector, the client and carer.
