Job OverviewAn exciting opportunity has arisen for an experienced Social Worker to join the Hospital Discharge Team in Newham
.
This ongoing, full-time, hybrid locum role offers a competitive hourly rate of £32.00 and the chance to work in a fast-paced, multidisciplinary environment where you will play a vital role in ensuring the safe, timely and effective discharge of adults from hospital into the community
.
You will be at the heart of the discharge pathway, working closely with health colleagues, families, and community services to promote independence, prevent readmission, and achieve the best possible outcomes for residents

What You Will DoManage a caseload of adults in hospital requiring assessment, support planning, and safe discharge arrangements
.
Undertake Care Act assessments, reviews, and support planning using a strengths-based, person-centred approach.
Work in partnership with hospital staff, health colleagues, and community services to facilitate timely and safe discharge.
Complete Mental Capacity Act assessments and Best Interests decisions where required.
Contribute to DoLS / LPS processes as appropriate.
Undertake Section 42 safeguarding enquiries and participate in multi-agency safeguarding processes.
Coordinate and arrange care packages, placements, and services to meet eligible needs and prevent readmission.
Attend and contribute to multi-disciplinary meetings, ward rounds, and discharge planning forums
.
Work closely with families and carers
, ensuring they are fully involved in assessment and decision-making.
Prepare high-quality professional reports for safeguarding meetings, Best Interests meetings, funding panels, and Court of Protection where required.
Maintain accurate, timely, and high-quality case recording on the council’s case management system.

Mental Capacity Act 2005
Human Rights Act
Relevant statutory guidance and local policies
Contribute to service development, performance improvement, and strong multi-agency working
.
Participate in supervision, team meetings, training, and continuous professional development
.
Degree or equivalent qualification in Social Work.
Registered with Social Work England
.
Experience working within Adult Social Care and/or Hospital Discharge services
.
Strong working knowledge of:
Care Act 2014
Mental Capacity Act 2005
Safeguarding Adults procedures
Ability to work confidently in fast-paced, multidisciplinary environments
