Responsibilities

This is a full-time hybrid role for a Community Health Worker, based in Canton, OH, with opportunities for remote work. The Community Health Worker will be responsible for engaging with community members to identify needs, connecting them to relevant health and social services, and providing follow-up support. Day-to-day tasks include conducting outreach activities, developing community partnerships, educating individuals on health resources, and assisting with navigating healthcare systems.

The role requires effective communication to support clients in overcoming challenges related to health and well-being.

Qualifications
  • Strong communication and interpersonal skills, with the ability to engage effectively with diverse populations.
  • Knowledge of cultural and linguistic competency in working with underserved or minority communities.
  • Ability to conduct outreach, develop partnerships, and provide education on social and health resources.
  • Problem-solving, organizational, and time management skills, with the ability to manage multiple responsibilities.
  • Experience in community health, social work, public health, or a related field.
  • Basic computer skills, including familiarity with Microsoft Office and healthcare documentation systems.
  • High school diploma or equivalent is required; post-secondary education in a related field is preferred.
  • Certified Community Health Worker (CHW) credential or the willingness to obtain certification.
  • Experience working in social or clinical services is a plus.
  • Valid driver’s license and reliable transportation are preferred for community outreach activities.
