Senior Social Worker - Home Based Primary Care (HBPC)

Serves as a core member of the interdisciplinary HBPC team which is a specialized program under the administration of Patient Care Services. Serves patients who would normally have received their care from the Community Based Outpatient Clinics or those who have impaired mobility due to disability or functional limitation, inability to cope with the clinic environment due to cognitive, physical or psychiatric impairment.

The Senior Social Worker - HBPC will independently provide a comprehensive psychosocial assessment and integrates it into an overall plan of care in collaboration with the interdisciplinary treatment team, the veteran, family members and significant others, whenever possible. The assessment requires a home visit. It is the responsibility of the social worker, working within the interdisciplinary treatment team to develop a treatment plan with the veteran based on this assessment.
