Position Overview

Human Edge Health is seeking a Travel Physical Therapist Assistant (PTA) for a home care assignment in Mansfield, CT . This role provides outpatient and home-based physical therapy services, supporting patients in maintaining mobility, strength, and independence.

Responsibilities

• Provide physical therapy interventions in patients’ homes according to the plan of care

• Assist with exercises, mobility training, and functional activities

• Monitor patient progress and report outcomes to the supervising PT

• Educate patients and families on exercises, safety, and home modifications

• Maintain accurate documentation in compliance with agency and regulatory standards

• Collaborate with interdisciplinary teams as needed

Qualifications

• Current Connecticut PTA License required prior to start

• Minimum of 1 year home care experience preferred

• Strong interpersonal and communication skills

• Ability to work independently in home care settings

• Reliable, professional, and compassionate approach to patient care

Shift Details

• 08:00 AM – 04:30 PM

• 40 hours per week

• No on-call responsibilities
