We are delighted to be recruiting a part-time Advanced Practitioner (AP) (0.6 FTE / 22.5 hours per week) to join our established and experienced team.

As an Advanced Practitioner, you will work on-site in GP practices as part of a dynamic multidisciplinary team. You will assess, diagnose, and manage MSK patients at the first point of contact - with full autonomy and clinical support. Work onsite in GP practices. Operate as part of a multidisciplinary team (GPs, ANPs, pharmacists, mental health, etc.) See MSK patients at undifferentiated MSK presentations

This is not a hybrid or remote position. You will be embedded in your practice(s), working closely with GPs, receptionists, nurses, and other MDT colleagues to deliver integrated, personalised patient care.

You will have the opportunity to use or develop advanced skills supported by protected CPD time and structured supervision.

Main duties of the job
  • Assess, diagnose, triage, and manage MSK patients at first contact, taking responsibility for a complex caseload.
  • Request and interpret investigations such as Xrays and blood tests, and make appropriate referrals to support diagnosis and treatment.
  • Use advanced skills including independent prescribing and injection therapy to deliver comprehensive MSK care.
  • Work with Primary Care Networks and GP practices to develop and deliver specialist MSK pathways and support appropriate triage before secondary care referral.
  • Conduct comprehensive assessments for patients with chronic pain, persistent symptoms, and multipathology presentations using advanced clinical reasoning and investigation skills.
  • Provide accurate diagnoses and formulate tailored treatment plans for complex MSK conditions using highly developed manual and clinical assessment skills.
  • Collaborate with community teams, orthopaedics, and pain services to support integrated care pathways.
  • Manage patients with psychosocial or mental health needs, using social prescribing where appropriate.
  • Deliver supported self-management programmes that promote behavioural change, physical activity, independence, and reduced reliance on medication.
  • Provide personalised, holistic care that considers overall health, wellbeing, function, mobility, and employment needs.
  • Support patients to set meaningful goals and build confidence in self-management.

We pride ourselves on providing you with unrivalled day-to-day peer support, CPD, and training opportunities.

Each team member gets 4 hours of protected supervision and CPD time per week (equivalent to 27 days per year!!), including a weekly 1-2-1 with an HEE-approved FCP Supervisor.

We support our team members through the Roadmap via all routes - including the HEI taught route, HEI supported portfolio route, and the current independent portfolio verification process.

We fully fund external level 7 MSc courses such as Injection Therapy and Non-Medical Prescribing.

We have now mobilised 175+ PCN FCP services and know that moving into primary care or into an FCP post for the first time can be daunting. Through our experience of implementing a large number of FCP services, we go the extra mile to make sure both you and the primary care network are fully supported to make the service a huge success.

Job responsibilities

Clinical Practice

  • Assess, diagnose, triage, and manage patients, taking responsibility for the management of a complex caseload. You will progress and request investigations when trained to do so (such as x-rays and blood tests) and referrals to facilitate diagnosis and choice of treatment.
  • Develop and make use of your full scope of clinical practice, including skills relating to independent prescribing, injection therapy and investigation when trained to do so.
