Division/Program SummaryThe Office of School Health (OSH) is a joint program of the Department of Education and the Department of Health and Mental Hygiene responsible for promoting the health of 1.3 million school aged children enrolled in approximately 1,800 public and non-public schools in New York City. This is achieved through a combination of public health initiatives, case management, education, and direct clinical services.
Asthma Case Management Program (ACMP) is a comprehensive school-based initiative embedded in the OSH Medical Unit that provides individualized care coordination, education, and follow-up for students with uncontrolled asthma. The program partners with families, school staff, and community healthcare providers to reduce asthma-related emergencies, improve daily symptom control, and support student attendance and academic success.
Under the supervision of the Asthma Program Consultant Social Worker, the Asthma case managers will be expected to provide health education, support and referrals for families of students with asthma. Communicate to OSH and community providers on asthma management and social services needs of students and families.
Job Duties and Responsibilities- Provide direct education to students and families on asthma control measures.
- Participate in case management meetings with OSH physicians and nurses to consult on student and families enrolled in the program to ensure all needs are being addressed and services coordinated.
- Provide appropriate referrals for families for social support, pest control and resources.
- Lead educational sessions for families and school staff as needed.
- Work closely with school and community-based organization staff on family engagement and school-wide education on asthma.
- Conduct outreach and community engagement activities.
- Communicate to gather and share information with external providers and partners to better coordinate care for students.
- Record all activities in the electronic student health record (ASHR).
- Staff must work in the Bronx.
