VHA's Physiotherapist is responsible for providing physiotherapy consultation, assessment and planned interventions to adults, geriatric and paediatric group of clients in their homes, schools and/or daycares across Southern Ontario and the Capital region, referred through VHA Rehab Solutions and Champlain Rehab Solutions. The physiotherapist will work within a continuous quality improvement framework that reflects VHA’s vision and mission of services. To promote and maintain good public relations with clients, customers and VHA staff, volunteers and associates.
** Key Areas of Accountability
** VHA's Physiotherapist have the following key areas of accountability, but not limited to:
* Assesses clients using interviews, standardized and informal assessment techniques in accordance with professional practice guidelines and standards established by VHA.
* Implements client interventions based on assessment information, client input and in conjunction with the client’s integrated care plan.
* Ensures that services provided are best practice, evidence-based and evaluated through the routine use of valid and reliable outcome measured.
* Attends client related meetings to report on assessment findings and discuss plans for intervention, progress reports and discharge as required.
* Ensures coordinated delivery of care through ongoing communication with all members of the client’s team regarding plans and schedules for interventions.
* Documents all assessment findings, treatment plans, progress and discharge summaries of interventions as required b the profession, referral source and the organization.
* Prepare statistical records of client care and related activities as required by VHA, the Ministry of Health and other agencies.
* Adheres to the code of ethics of the profession, in maintaining standards of practice for the profession as defined by the Ontario College of Physiotherapists.

* Required Qualifications:

*** Baccalaureate or Masters degree in Physiotherapy from a recognized school of physiotherapy.
* Registered, in good standing, with the College of Physiotherapists of Ontario.
* Minimum of 2 years clinical experience, preferably in the community.
* Specialized courses to support areas of clinical expertise.
* Clinical expertise to provide a variety of physiotherapy interventions.
* Knowledge of community care requirements and resources available.
* Excellent consultation and teaching skills, and utilizes an evidence base to clinical practice.
* Familiar with the routine use of valid and reliable outcome measures to support clinical interventions.
