- Assist with and coordinate care management activities under the direction of a case manager or supervisor
- Communicate with healthcare providers to arrange follow-up appointments and obtain clinical records
- Document and manage the care management process, including case identification, assessment, and planning
- Collaborate with members’ primary care providers and community resources to facilitate care coordination
- Provide behavioral health support and interventions to members in crisis
- Bachelor’s degree in social work, Practical Nursing Certificate, or equivalent degree in a relevant healthcare field
- Minimum of one (1) year of clinical and/or care management experience
- Proficiency with Microsoft Office products
- Knowledge of Medicare and Medicaid regulations
- Experience in care management within a health plan, community agency, or Accountable Care Organization (ACO) is preferred
- Previous experience as a medical assistant, behavioral health technician, or in social welfare services
- Knowledge of care management systems and planning techniques
- Experience with care management workflow systems
- Strong understanding of medical terminology and healthcare documentation standards
- Comfortable presenting in group settings
