Colorado Mental Health Hospital in Pueblo (CMHHIP) is a 24 hour behavioral health acute care facility and a cornerstone of the Pueblo community. CMHHIP provides a safe, secure, and therapeutic setting for patients with psychiatric and medical needs, with the goal of returning patients to their optimal level of functioning and reintegrating them into the community as appropriate.
Position DetailsDepartment Information:
Registered Nurse I – Full Time, CMHHIP. This position has a residency waiver and will be accepting applications from residents and non-residents of Colorado. Applicants from outside of Colorado must relocate to Colorado prior to their start date at their own expense. A short-staffing temporary pay differential of 4% will be in effect from July 1, 2025 to June 30, 2026.

- Assist with the implementation of the CMHHIP Critical Incident Reporting Program as outlined in CRS 25-1-124 (2).
- Review and analyze incident reports using standards of practice from CDPHE; perform a fact finding assessment to determine critical incidents and reporting requirements to leadership and regulatory bodies.
- Collaborate with Unit Leadership, DPS, and CMHHIP leadership to respond to serious allegations and ensure patient safety.
- Participate in Root Cause Analysis (RCA) and Apparent Cause Analysis (ACA) to guide quality improvement strategies.
- Maintain knowledge of CMS, TJC, and CDPHE standards and related statutes; assist in onsite survey activities and policy/procedure updates as directed.
- Provide clinical judgment and nursing expertise as an occurrence reporter, with responsibilities including data collection, analysis, and documentation for regulatory reporting and quality improvement.
- Prepare and submit RCA/ACA reports, ensure differentiation between facts, professional judgments, and recommendations, and maintain data in required databases.
- Utilize investigative tools (interviews, video review, documentation) and prioritize incidents when multiple investigations occur.
- Coordinate with leadership to respond to investigations and formulate quality recommendations for operational improvements.
- Other related duties as assigned within the scope of practice for this position.
- Report writing and data entry for ACA/RCA investigations; ensure comprehensive and compliant documentation.
- Maintain confidentiality of incident-related and quality management documents in accordance with regulations.
- Participate in video audits and on-unit wellness checks as directed by Nurse Manager or Department Leader.
Must possess a valid, active Registered Nurse license issued by the State of Colorado (DORA) or any state participating in the Enhanced Nurse Licensure Compact (eNLC).
Preferred Qualifications- General clinical experience in an acute care setting; experience in behavioral health is ideal.
- Root Cause Analysis and/or Incident Management experience preferred.
- Clinical assessment skills and experience with incident reporting systems; ability to investigate occurrences and determine reporting criteria.
- Data collection and analysis experience; familiarity with quality improvement initiatives and QAPI collaboration.
- Familiarity with Joint Commission and CDPH rules and reporting requirements.
- Ability to see and hear to observe safety and security; capable of working in situations involving assaultive behavior and restraints.
- Physical ability to lift up to 50+ pounds; ability to perform in demanding environments and travel as required.
- Full background checks required, including fingerprints and eligibility screening for direct contact with vulnerable persons; CAPS check for at-risk adults as applicable.
- Shift flexibility, ability to report for essential/emergency services, and completion of required training (including CMHIP orientation, Defensive Tactics, and CPR).
- Influenza vaccination is expected annually.
